Across the U.S., community-based organizations, hospitals and clinics, and government agencies are advancing health equity through innovative approaches to providing healthcare. Capitalizing on their shared commitment to address health inequities in organized ways, trained practitioners and volunteers share a vision to achieve equitable healthcare for all.

Here are some representative stories drawn from a much larger and complex set of collaborations. These examples are taking place in communities where resources and strategies are used to address local conditions and barriers with great success.
