Healthy Neighborhoods map
Title
Healthy Neighborhoods map
Description
Innovation defines the City of San Antonio’s Healthy Neighborhoods program and its approach to deploying its community health workers (CHWs). Launched in 2012 to improve health at the neighborhood level, the program believes that all families deserve healthy neighborhoods and that they have what is needed to create positive change. To channel these strengths, the program uses neighborhood people to serve their communities as CHWs or promotores de salud.
The Healthy Neighborhood program’s community connectors—as the program’s community health workers are known—approach their work in ways that differ from more traditional CHW programs. The connectors are empowered to develop community programs based on the assets and needs of their communities, as well as their own strengths and interests. Currently, 15 San Antonio neighborhoods participate in the Healthy Neighborhoods program.
